Insurance Information
The State of California allows direct access to physical therapy services, but if you are choosing to bill your insurance company, we require a doctor’s prescription. We bill most insurance companies directly. We are contracted with and accept Blue Cross, most Blue Shield plans, Humboldt IPA, Medicare, TriWest, and many Workers Comp insurance companies. We do not take Tricare, Partnership/Medi-Cal, or any motor vehicle clients.
Please understand that when choosing to use your insurance for our services, extra time and staff resources are required. It takes time to coordinate a referral from your doctor’s office, time to verify your benefits, and more time still if your insurance requires authorization. We are happy to take these steps to bill your insurance for our services but be advised these steps generally result in longer wait times to get on the schedule than for cash clients.
Josie Medrano is an integral part of Hands-On PT. She is the person who will coordinate your care. She will kindly and efficiently assist you with any questions you have about our office. She works to coordinate your referral from your doctor’s office, she connects with your insurance company to verify your benefits, and she handles all scheduling. She is knowledgeable about the services we provide and the policies of the practice.
Intake Forms
To enjoy the most streamline evaluation appointment please fill out the online forms and email them back to us a day before your appointment. You may notice some forms are working documents that you can fill out and email back while other forms needs to be printed. If you choose not to use the online forms, please arrive 15 minutes before your appointment time to complete the intake process.
Cancellation Policy
As a way to maintain a respectful client and therapist relationship and to value everyone’s time, we choose to charge for missed appointments and late cancellations. We require 48-hour notice if you need to cancel. We like that much time so we can get another client on the schedule. Fee is $25 for 30-minute appointments and $50 for 55-minute appointments. It you have an insurance that does not allow for us to bill for missed appointments, you may be discharged from the practice after the first late cancellation or missed visit.
Billing
We coordinate with Precision Billing, an outside billing company. Josie will be able to answer some of your billing questions and when she can’t she will refer you to Judy Judge at Precision Billing.

Rates for self-pay Physical Therapy
Evaluation: $160 for 60 minutes
Pelvic Health Evaluation: $175 for 60 minutes
Follow up appointments: $120 for 55 minutes
